The Occupational Safety and Health Administration has levied a fine against a construction company after a worker fell to his death from a Jackson work site last year.

Stacie Washington, 43, died at the University of Mississippi Medical Center a few days after he fell five stories. He had been working at a construction site at Baptist Health Systems on State Street in Jackson on Oct. 12 when his harness broke.

OSHA inspected McPhail Construction the day of the fall. According to OSHA they found that McPhail didn't provide workers full protection equipment at no cost, and that equipment was not inspected prior to each job. OSHA says the problems have been corrected.

McPhail Construction received the citations Feb. 27. The company has a chance to contest the $9,240 fine before it's implemented.
