Deaconess HomeCare offers a broad spectrum of specialized healthcare services, meeting the needs of adults and children in the comfort of their own homes. Patients are referred for our services by their physician. A patient, caregiver, friend, clergy, or health care provider can call for information. Services are available 24 hours a day, 7 days a week.
With physician referral, we offer:
Skilled Nursing, including speciality areas of Enterostomal Therapy, Oncology Nursing, and Infusion Nursing
Initiated by a physician's order, our nurses take care of dressing changes, medication assistance and can even provide education about your medical condition to help you manage your condition successfully. An individualized plan of care is created to serve each specific need.
Before services are started, an RN assessment is completed to assess the person's medical history, psychosocial and physical status, home environment and safety.
Deaconess HomeCare promotes optimal safety in the home. During the initial visit, the RN will make appropriate suggestions or referrals to keep your home safe. See our handy Safety Checklist to evaluate your own home.
Physical Therapists provide restorative and rehabilitative services.
Speech Therapists provide speech and language therapy.
Occupational Therapists provide restorative and rehabilitative services focused on enabling the patient to perform their activities of daily living.
Medical Social Workers
Medical Social Workers identify needed and available resources to allow optimal medical recovery.
Home Health Aide
Our home health aides assist with daily tasks, such as bathing, grooming, dressing, hair care or assistance getting in and out of bed.
Staff is available by phone 24 hours a day for emergencies or to answer any questions that may arise.
